To schedule an appointment please contact us at 402-483-4485 Monday through Wednesday 8:00am - 7:00pm and Thursday through Friday 8:00am - 5:00pm.
Please arrive at least 15 minutes prior to your appointment to complete the registration process.
Please bring the following items to your appointment:
- Current insurance card (s)
- Photo ID
- HMO/PPO referral (if applicable)
- Worker's compensation insurance information (if applicable) including insurance carrier, information and address, authorization letter, claim number, adjuster's name and telephone number.
- If a patient is a minor (18 years or younger), a parent or guardian must accompany the patient to their appointment, or the patient must have a written consent from a parent or guardian in order to be seen by a podiatric physician. All patient forms must be completed and signed prior to the appointment by the parent or guardian.
